What is Mi Community Care?

MI Community Care is a free community-wide care coordination program run by regional health, behavioral health, and social service organizations. Hundreds of Livingston and Washtenaw County residents have benefitted from the program, which began in 2016 with federal funding. Participation is free of charge and does not jeopardize other forms of aid.

what's next

HOW DO I GET STARTED?

Talk to someone—a health care provider or a social service agency—about getting a referral to the program. Show them our MiCC provider handout and referral form.

Answer some questions so your MiCC care coordinator can understand your health and life goals, then work with your care coordinator to make a detailed plan.

Sign a consent document so your care coordinator can talk to other agencies (you can decide which agencies) to help you meet your health and personal goals.

Meet with and talk to your care coordinator on a regular basis—you can also talk to a community health worker, a peer support specialist, a doctor, or others, as needed. Actively participate to achieve your health and life goals.

Get health insurance, prescriptions, medical equipment and scheduling appointments with providers

Housing

Search for safe, affordable housing, including supportive housing when needed

Food and Transport

Access food and home-delivered meals and get transportation to appointments

Mental Health

Accessing mental health or substance use support when you're ready to take that step

Financial Resources

Get help with finding funding for electricity, heat, water, medical bills, debt

Accommodations

Get furniture, home maintenance, or home modifications to meet your health needs

my care coordinator

Once your health goals are identified, your care coordinator will help you achieve those goals. For instance, if you have a chronic medical condition, need home-delivered meals, need transportation to appointments, and need help filing applications for support, your care coordinator will coordinate with colleagues, troubleshoot problems, and help you overcome barriers to meeting your goals.
